Pitman Animal Rescue Foundation is the parent organization for the Dachshund Rescue of Utah (DRUT) and the Dachshund Rescue of Los Angeles (DRLA). DRLA is our sister rescue. Your payment will go to DRUT.

Confirmation of Terms and Conditions of Adoption

This Adoption Contract is entered into between Dachshund Rescue of Utah (DRUT); a part of the Pitman Animal Rescue Foundation/Dachshund Rescue of Utah (hereinafter referred to as "DRUT") and the new owner listed above (hereinafter referred to as "Adopter"). DRUT is committed not only to the welfare and protection of the dog, but to your satisfaction as well.

(Check all boxes that you agree to) In order to ensure these objectives are met, DRUT and the adopter jointly agree to the following: *

• DRUT agrees to provide the dog with appropriate vaccinations for DHHP (Distemper, Hepatitis, Parvovirus and Parainfluenza), Rabies and Bordetella, providing appropriate certification thereof. DRUT agrees to spay or neuter the dog prior to adoption OR to hold a security deposit of up to $100 until acceptable documentation is received stating the dog has been spayed or neutered within 30 days of the adoption. Failure to spay or neuter the dog nullifies this contract; in the event this agreement is violated, the dog must be retired to DRUT/PARF/DRLA. No donation refund will be given.
• I/We hereby agree to care for this dog humanely, including adequate food, water, clean and dry shelter and daily exercise. I/we agree to provide all veterinary care and treatment the dog needs, including annual medical checkups, dental treatment, and veterinarian-recommended vaccinations and preventative heart worm medication, as applicable.
• I/we agree to keep this dog as an indoor house pet and companion, to be exercised ONLY in a fenced yard or on a leash and NEVER to be free to roam the streets. I/we agree not to keep this dog on land or rented property where a "no pets" policy is in force and to obey all applicable laws governing license, control and custody of animals in the jurisdiction in which I/we reside. I/we agree to provide a safe collar with a Rescue Tag and an I.D. tag with our address and telephone number.
• I/we agree that this dog shall NOT be used for medical or other research or experimental purposes. I/ we further agree that this dog shall not be chained or tied and will never be used for breeding. I/we further agree NOT to sell, trade or otherwise dispose of this dog in any way, but to RETURN the dog to DRUT if at any time I/we wish to relinquish custody. I/we agree under such circumstances to give DRUT three weeks of notice of the necessity for them to take the dog back into rescue.
• I/we understand that a representative of DRUT may visit my premises at any time and may examine or make inquiry about this dog to ensure that the terms of this agreement have been and are kept. If the representative is not satisfied with the condition of the dog or the condition tin which it is being kept, the dog may be reclaimed IMMEDIATELY by DRUT without further notice. I/we hereby give my/our permission for an agent or representative of DRUT/PARF/DRLA to remove the dog from my/our premises and stipulate the such entry shall not constitute a trespass upon the premises occupied. I further agree to pay all costs, including attorney's fees, if DRUT must take action against the Adopter or his or her representatives or agents to enforce any of the terms of this contract.
• It is understood that DRUT/PARF/DRLA cannot be responsible for the conduct of this dog after its adoption, and that any interaction between this dog and any adult, or minor child is solely the responsibility of this dog's new Owner. I/we agree to accept any and all liability for, and to take steps to prevent any destructive behavior on the part of this dog.
• Adoption fees and deposits are non-refundable, as the fee is used to treat the next needy Dachshund. A replacement dog may be provided depending on the circumstances of the return and shall be at the discretion of DRUT.
